Is the LED waterproof/rain-proof?

Not only is it fine in the snow and rain - mine survived a full cycle wash in the washing machine. (I forgot to remove it when the hat went in the wash) Left it to dry for a couple of days, plugged it in to charge it up and hey presto it charged up and has been fully functioning ever since.

Does the beanie come in a box with explanation on how to use it?

it comes with a clear bag, and it very easy to use, just press the power button in the middle of the LED, and get the LED off to plug the USB port for Charging. Hope this can help you.

Is it suitable for 4 year olds or will it be too big?

I bought one for my 5 year old’s birthday & he wears it a lot. Fits him very well.

How do I know is full charged? Light shows in red no flashing

The red light turns green, it means that it is fully charged. 1-2 hours. Hope this can help you.
