Q: has anyone experienced the adhesive failing and than everything comes crashing down?

No ours are stuck solid , mine are loaded right up too. We let them sit for 48 hours once stuck to the wall to allow the adhesive to work and settle

Q: Can we fix these to painted walls without tiles?

Yes I have fixed it on the plain wall without tiles.

Q: does it leave stains or damages to in the wall after removal?

Sorry I can't answer that question as I haven't took it off the tiles
