Do you know if it had been acid washed or naturally activated?

I suggest you contact the seller / supplier, as I don't know. There is nothing on the packaging to indicate how it has been activated.

Can I use this for my plants

I use these in my terrariums and it works fine.

Does it need washing before i put in my filters?

I generally rinse this product through the water I take out when doing a filter change as there is quite a lot of fine black dust in with the packaging

How often does this carbon need replacing in an aquarium filter?

We do ours every 4-6 weeks but we only have about 14 small fish. If you have bigger fish you would need to change it more often!
