Definitely a medium! It shrinks a bit after a wash.

Not sure about a 4XL but I ordered an adult S and it is about 2X smaller than a typical size Small shirt. The quality of the material is not great either, I have not washed the shirt yet but it looks like the type of material that will shrink even smaller.
