Q: What are the holes on the side used for?

I used those holes to make my own key chain

Since Altoids come in a couple different sized boxes, I'd rather err on the side of caution and give you the dimensions. Folded down the pocket vise is less than 2.5 inches long. It's also about an inch wide so you can probably calculate that one or two would fit.Keri-Lynn, Four Seasons Survival

Q: Do you have to take it off the keychain to use the bottle opener?

I haven't used the bottle opener but I think it would be difficult to do with a bunch of keys attached to it. Its really small and more of a novelty. The spring loaded carbener latch kinda sux and it falls of the keys a lot. But what do you expect for $9

Yes it is a no.2 size sort of. I placed it next to my regular screwdriver and the size and angles seemed to be similar. The bit is a modified version of the Phillips driver and while ok in emergency I would not advise it as your tool for day to day work.

Delivered on time.

The metal finishing is sharp-edged. The shear work very well but the 2 blades open toward the key ring, which reduces their effective use. The bottle cap opener is very small.
