Q: Which side is placed up?

We placed the spongy side down, and the plastic coated side up. If you place that plastic on wood or carpet then over time it can stick and do damage. Plus the sponge is what helps it grip.

Q: Would this add enough cushion for a crawling baby under a flat cotton rug on hardwood?

This would be great for your grandkids. I purchased a very soft rug for my great room for my grandkids to play on but it needed a little more cushion. I purchased this cushion and it is great. They drop their entire bodies on it and lay all over it. This cushion would be ideal.

Q: Is this pad water proof

Yes it is. My husband spilled a full glass of water and it never made it to my new wood floor.

Q: will this prevent slip on hardwood floor? Also does it have any smell? want to use in toddler play area

No, it still slips on my hardwood floor. I would recommend also purchasing adhesive or velcro strips to hold in place.
