Q: Can I put 4 insulin pens in it and how long long does it stay cold?

I was able to fit only 3 injection pens into the tri-fold with the 3 ice packs. I bought a separate ice pack that fit into mesh pocket. It all kept the pens cold in my carryon luggage during a 7.5 hr transatlantic flight. The temperature strip on the tri-fold wasn’t readable. The 3 ice packs were thawed but cool and pens cold to the touch. The large extra ice pack I bought was still a big frozen and it’s because of that one extra pack that I think my insulin made it that long.

Q: Will hold 3 Trulicity doses?

It will definitely hold 3 typical syringes, but I'm not sure how large a trulicity pen is. The dimensions of the cooler travel case are given in the description.

Q: How cold can the ice pack get? Is it like a regular ice pack?

They are regular little ice packs.
