is this one size fits all?

I would say one size fits most. My feet are really small and I was able to adjust the supports to fit me. I don't know how they would fit someone who has rather wide and or large feet

How often do you have to replace with a new pair?

I don’t know the answer but they are very well made. Well worth the cost, and they work amazingly. The only drawback is they are too thick with my shoes.

Mine only came with the gel inserts in the product,and 1 small, 1 medium in clear baggies.The copy reads 6,how do I obtain my missing inserts?

I thought I only had one of each, there were two they were just stuck together
