Never plant an orchid in dirt. Perfect way to kill it. Use some good bark. The fertilizer is to be used weekly.

Q: At what growing stage are you supposed to use this? All the time?

Depends on what type of orchid and the season. During growing season, every 3-4 weeks is a good average. Back off after flowering, as some orchids have a dormant stage

Q: Good for amaryllis ? All bulbs ??

I would use 24-8-16 much like miracle grow
