how to choose the size?

The insoles are made so you can cut them to your shoe size, per example the one that says size 8-14 it fits for all those sizes and when you open them you will see a line that shows the correct size for you, I am size 8 so I had to cut the extra part of the insole to feet my shoe.

Can these be put over top of original sole? Or is it to be replaced with this?

You can do either. If the original insole has some cushioning and you want the most padding then leave the first one in. As a side note these will probably need some trimming around the outer edge to fit it in the shoe. Hopefully this will help answer your question.
