Do you remove the foam in the lid to install?

My Pelican did not come with the foam option. My guess is that you’ll probably have to remove the foam to expose the premade holes on the inside of the lid. Those hole were meant for the mesh organizer to be installed as an option.

Can the pouches be removed and repositioned?

No, pouches are permanently attached.
