what size hole do you need to cut? how far apart are the mounting screw holes (center to center)?

1-7/8" minimum. A 2" hole will work great too. Three holes spaced 1-11/16 in an equilateral triangle for mounting with one hole directly in line with the ground prong.

I have noticed a few reviews saying the cap won't stay in place. Has anybody identified a way to make the cap stay in place?

I have found the solution is to find a washer the same diameter as the inside of the cap. insert washer in cap - should be a tight fit - close cap - cap stays closed.

How is the inside configured meaning is there a female receptical for a plug inside the trailer

No. The inside is configured to accept wiring. I used this to feed a second air conditioner on my camper. Just attach the female end of an extension cord to this and plug the male end into a 120 vDC receptacle.

Can this be used on a home as a generator inlet?

I have no idea. It is rated for 240 AC up to 20 Amp.
