How can i determine which bushing kit to purchase? my old shock is a fox drcv float rp2 (which had a wider head mount at the top)

Make sure you get RS mounts, but the one you need matches the size of the mount on your bike. They will come with spacers. Just measure the mount and purchase accordingly.

Does it come with hardware?

It came with only the shock shown in the photo. You will need to buy two of the RockShox mounting hardware kits that are $11.50/ea to complete the install.
