Is it waterproof and snow-proof? Our 500 gal tank is only 6 or so inches above ground in Wisconsin. It will get snow covered and rainwater.

Yes it is waterproof. As a part of the "Class 1 Division 1" certification for dangerous/explosive environments, it also gets IP tested for water leaks and such. All the electronics are fully encapsulated except for the batteries. But then the outside of the entire enclosure has a rubber seal there as well. As a tip, make sure to use lots of ultrasonic grease (included). That of course helps the reading have a stronger quality, but it also helps to prevent water from working its way between the sensor and the tank, then freezing and getting a large layer of ice between it that could impact the quality of the readings. The PRO+LR has significantly stronger wireless, but if you have trouble with wireless range when buried under the snow, you can also attach a wired antenna to move out from too much snow. Any 2.4Ghz wired antenna on Amazon should work (same frequency as WIFI antennas). Just look for an "SMA" antenna and make sure it has the "male" pin on it just like the included antenna.

Is there a monthly/yearly monitoring fee when using the app?

No there's not a monthly monitoring fee with the app. Note that you may see some references to a monthly fee in certain documentation, but that is for our cellular version and dashboard, which is not currently sold on Amazon and only direct to businesses.

On 120g tank can get hand in bottom but may not be far enough to position in btm center. Will this still work correct?

Unfortunately its hard to say - the sensor needs to typically be within about a 5 degree incline for best performance. The sensors will stick on with the magnet which should help, and then you can likely slide it in to place with a stick while watching the bubble-level in our app. The sensors are 4.2 inch diameter and 0.63 inches tall (16mm tall). Are you able to post a picture here in questions, or else maybe try clicking our "Mopeka" seller id somewhere and finding the button that says to contact us? I don't think they let me give out links or contact information outside of Amazon here in the product questions.

Does the Mopeka PRO+ LR wireless operate on wifi AND bluetooth out of box? And does this require a monthly subscription to use?

As far as I know, this unit does not operate on Wi-Fi. Bluetooth only and there is no monthly subscription. It goes directly to your phone or tablet. I can't get it to connect to my computer and don't know why. Hope this helps.
