You don't have to use one but I would use a snorkel.

Would this be good for a 4 year old?

That would depend on their current level of technical proficiency. This float is less stable & offers less support (buoyancy), & is typically for single-handed usage (although I suppose because of it's relative dimensions & the hand size of a 4 yr old it could be used 2 handed like a conventional float?). Suffice to say: If your child is quite technically advanced this float would be an appropriate technical training aid, otherwise go for a more conventional (rectangular) float.
