It´s good for a men shoe size # 15 ?

No. The medium size will not fit a mens size 15. I have a size 8 womens and used this when I hurt my foot. It will not fit my boyfriends foot and he is a mens size 9.

Is this covered by insurance?

Not through amazon you would have to go through a specialty store that have them stocked but to be honest I doubt Insurance pay for it I had to pay out of pocket
