Q: What is the diameter of the opening hole, from front to back? I need something big enough for a 5yo who's just a little too small for a standard seat.

I think this would be too small, It just fits my 2-1/2 granddaughter

First thing to answer this is you would need to speak to a doctor before doing that. Honestly though, this toilet seat is meant more for smaller children during potty training. It would not be good for a teenager or adult, etc. These are meant for kids 3-4 years old, maybe 5 but that's pushing it. A normal toilet seat is too big for a small child, so that's what this is for as well, to accommodate a small child using the restroom.

Q: How do you keep this from sliding around on an elongated seat? It tries to slide to the very back of the toilet seat and rotates part way too.

Very unsafe. My daughter fell forward and hit her head, then had to go the doctor. Generally don’t find the seat secure.
