Will this fit a large size man for back ??

If he has a large waist, you will have to get an extender that has velcro attachments. It's small for my 40" waist, just barely fitting.

How long does it stay cold?

I didn't time it. However I needed to ice after surgery for several days and I thought it stayed cold a pretty long time.

Does what it says on the pack !! It's re-usable and holds the ice-cold for a long time. I need it for my lower back and it's not exactly the right size and shape - but it works okay. The best I could find - and I searched long and hard !!
