Does this plug fit inside the receiver or just covers the square entrance? Can you run the hitch pin through it?

It's a cap that covers the entrance with the plug being a friction fit inside the receiver but not so deep that a hitch pin will retain it. One came with the receiver of my 2006 Tacoma and was only lost February 2018 when the truck was rear-ended, so the friction fit seems fine to retain it in normal service.

Will this fit for a 2018 Tacoma ?

Yes, absolutely. It is a push fit into the receiver and not secured by any hardware. Mine has been in place since I bought it for my 2018 Tacoma TRD Sport. I had one in my earlier 2006 Tacoma (same receiver size) for years - the push fit is very secure.
