Q: What size of the nipple that comes with the bottle ?

They come with 2 slanted slits in the top. It reduces the flow slightly but you can enlarge the slits if need to increase flow easily.

Q: Is the flow suitable to 20 months old? My son takes level 3 dr browns nipple. Is the flow faster than that?

Maybe a little faster, but not much.

I didn’t like this product and returned it.

Q: How does the spout work? We can't get anything out of these.

There is a tiny slit, the baby has to suck on it. You can also turn it upside down and squeeze the spout and a small amount of milk will come out. It’s a slow, but great transition from regular baby bottles to this so they don’t chock.
