How is the length measured? Just the threaded part or the entire bolt (head included)?

The length is measured by the length of the shaft of the bolt. From below the head to end of threads.

Will these get surface rust if used for outside?

Eventually it will rust as does any steel related product. However it should be quite so time before it does. I live in Tennessee and used these fasteners on my lawn furniture which we get rain and humidity and they have not rusted yet.
