Would you rinse before or after brushing? Or both?

My husband uses it after brushing and in between brushing if he wants to freshen his breath. Another great product for in between are lozenges call Thera Breath. They are excellent at moistening the mouth and are sugar free. Out dentist recommended them.

This is double the price of what this costs at almost any store (for the same size bottle). Is there a reason for this?

No, there isn’t. I can get it at Walmart for half the price.

Does this burn like Listerine does? Bottle states mild, but one review said strong / bad taste.

No it does not like Listerine at all. I’ve used both and find Biotene Oral Rinse much better in taste and does not burn. I’d be careful in buying the one that says Mint flavor or something like that. Unless of course you don’t mind the mint taste.

Is this mint flavored? Ingredients just say "flavor". Dentist recommended it for my husband but he has a mint allergy.

It doesn't taste mint flavoured to me, but it doesn't very good. Swish and expel fast!
