Can this be used as an outside faucet? Obviously would have to shut off in the winter, but would it withstand the elements?

It is meant for inside. It is all brass, so I would think it would withstand elements. It would certainly weather in appearance, and I would expect some rust from the bracket.

In the center of the spout it looks like there's some sort of cap, what is this? Is that "cap," and the entire mid-faucet piece, standard pipe thread?

That cap is a safety feature ,it acts like an air gap.
