Like Patrick said, if you have mechanical issue - see mechanic. Try this every oil change. If the valves are sticking due to sludge it will help clean moderate sludge deposits.

Some engines Must not be overfill. This product was created to add to whatever amount of oil exists in pan, but not doing so with an entire quart. I have a car that must not have more than 3.8 quarts, making this perfect to use each oil change; 3 quarts of oil and this smaller bottle of Rislone, once at operating temperature it is just below the full mark!

I would think it all depends on the reason the lifer or engine is ticking. If the lifter is ticking from dirty or plugged oil passages being this is designed to clean things up you may experience less or no more tick. If the noise is from actual valve or lifter / engine damage ( from over heating etc) or from valve trains that require regular adjustments and that service has not been done, I would not expect any oil product would fix that. However, based on the many positive reviews suggesting this product has helped valve tick and based on the low price of this product I myself would give it a shot before attempting an expensive engine repair.

No, do not add when oil is full, you should never overfill any motor. It is an excellent product to add when oil level is low, it will restore depleted oil additives.
