Can it give a false positive if the sample is mixed with blood?

There's nothing in the blood to make it react. The test looks for acid phosphatase which is in seminal fluid in high concentrations, while other body fluids, like female secretions have very low concentrations.

Will this work if the semen is dried on the clothing?

Yes. It's generally used on dried semen stains. Sterile swabs are included. You moisten one with water and rub the stain to transfer the stain to the swab. You then take the moistened swab and place it on the test strip. If it turns bright purple in 15 seconds, it's positive

What if someone uses ammonia to destroy the semen?

If its been destroyed there is nothing to test. But how do you know it is destroyed? Do a test on a semen sample that you clean away with ammonia. This will tell you if it is test-able. If it is positive there will be a bright purple color that appears quickly.
