What is the difference between this and the regular one, what does ultra do extra?

So ANNOYING that ingredients are NOT LISTED!

Though there are FOUR product shots of the front of the package from slightly different angles, there are NONE of the BACK which would show what is in them. Nor are they listed in the product description, which states “Ingredients: see packaging for ingredients”

The short answer to your question is that Ultra also has aminomethyl propanol—which is a buffer that neutralizes acids.

Here is what I found on-line, and which I did cross check and so believe to be correct—no guarantee though.

Systane Ultra:
Active ingredients:  Polyethylene Glycol 400 0.4%, Propylene Glycol 0.3%
Inactive ingredients:  Aminomethyl propanol, boric acid, hydroxypropyl guar, POLYQUAD® (polyquaternium-1) 0.001% preservative, potassium chloride, purified water, sodium chloride, sorbitol. May contain hydrochloric acid and/or sodium hydroxide to adjust pH.

Systane:
Active Ingredients:  Polyethylene Glycol 400 0.4%, Propylene Glycol 0.3%
Inactive Ingredients:  Boric acid, calcium chloride, hydroxypropyl guar, magnesium chloride, polyquaternium-1 as a preservative, potassium chloride, purified water, sodium chloride, zinc chloride. May contain hydrochloric acid and/or sodium hydroxide to adjust pH.

We ordered both Ultra and Balance to see which one works out best for us. That is one of the things about eye drops—individuals, even with the same symptoms, may have different responses.

Is this preservative free?

A Systane bottle containing MORE than one dose in it has "POLYQUAD (polyquaternium-1) 0.001%" as a preservative. (With any medication, all multi-dose eye drop bottles contain preservatives.) The single use Sustame vials are preservative free.  However, even though I usually have a sensitivity to eye drop preservatives, I can use the multi-dose bottle of Systane drops without any trouble. I did an experiment where I used drops from a Systane multi-dose bottle in one eye and drops from a Systane single dose vial in the other eye.  After 2 weeks, there was no difference in my eyes!

Would you recommend this for eyes that are dry and tired after computer strain?

Yes, very much so. They were recommended to me by one of my eye doctors for MY dry eyes. Most of the over the counter eye drops contain alcohol, or some other drying agent that initially feels good, but after a little while your eyes become as dry or dryer than they were before using them. These Systane drops do not contain any of these drying agents---alcohol or other. It is also better to use these more often than not, as dryness can & will create minute scratches on the eye-- again--as told to me by my eye doctor.
