what packaging does this arrive in?

It arrives in a cardboard box. Packaged well, mine arrived in perfect condition.

Can this tub stand on the floor or does it need to be in a shower stall?

For using on a non drained floor area. Say you got a crappy tub you don't wanna use, but wanna use this beside it and has enough space. You can use a portable shower pump (found on amazon here for around 20-40$ cheap.). Take the shower head off, and fill the non using tube with water till it reaches the suction head of the portable pump. Fill the blow up tube with the water. Easy. Do the same to drain, No need to drain using the bottom drain hole. You just reverse the pump to take the water from the blow up tub and poor it back into the main tub or shower or sink. But be aware this'll take probably 2x longer to fill/drain then having a shower head or flexible tub hose to fill the tub with, or to drain it on the floor with the bottom/front drainer holes.

Mine has two holes on the bottom of it, the part that touches the floor. One has a cover, the other doesn't. Is this right...?

Mine is the same way. An it’s been just fine so far. My only issue at all with it is my second from the top around won’t stay aired up. But doesn’t have a hole an is always placed correctly so who knows

Can a person with a knee injury get in and out of this?

It would be hard to get in and out with an injury. Plus it does not drain well, you would need a lot of room for this tub.
