It works with 12v power or must be with 220v?

The contact rating is 220v AC. It should work fine with 12v dc. Much as a 220v light switch could be used to switch 12v.

What does the screw cap look like on the opposite side of the gauge?

The gauge can be fitted on either side, a screw seals the hole on the other side to the gauge.

It’s not a pump. It’s a controller. What it does is switch on the pump when the pressure drops, and switches it off when water stops flowing and the pressure builds up.

Why doesn't this pump controller come with some sort of mounting bracket.

It is mounted on the pump directly & sits on top of the pump.
