Those definitely do not look like they are shearling.

Q: Question. i have your shearling clogs in 45 (eu) size - i’m a man - i want new ones. will the ‘women’s shoe fit me in a 12.5? or are these women only?

The size 12 - 12.5 is a mens size 45 EU, I don't know why Amazon's catalog team has the word "womens" in the description since this is a unisex clog

Q: I wear a size 5 in any other brand. these size 5 the arch support is in the wrong place and i can’t walk w/o pain. should i have gone with one size up

I wouldn't recommend going UP a size in these as your feet would likely slide to the front and maybe even over the toes. The band might even be looser. Where is the arch support hitting you? Perhaps a DOWN size? I'm a 9.5-10 in every shoe I wear (leaning closer to mostly 10s) but in Birkenstock, 10 is a 41 (10-10.5) which WAY too big for me. In my Arizonas and in these heels I wear a 40 (9-9.5). Once I wear them a bit, the cork molds to my feet and arch support is perfect.

Q: are birkenstocks unisex as in, for men if you buy a regular/medium width?

If the man had fairly narrow feet, these should fit. In my opinion, if his foot is wider, these may be too tight.
