Does this come with a hose for connection to the bath mixer tap?

Hi, yes, it came with two hoses, one for the mixer tap connection, and the other to connect to the shower head. The hose to the mixer tap was quite short, maybe 30cm.

Will this work with a bath mixer tap?

Yup - that’s what fine is hooked up to. Really nice bit of kit - does the job and looks the part.

How many holes or crews for this please to fix to the wall, as dont want to drill more holes than we currently have (we have 1 at top and 1 at bottom)

It needs 2 holes the lower one holds the valve to switch between the fixed shower head and the hand shower and is fixed solid to the wall. The upper positions the upper shower head but is only a support for the pipe the pipe is not fixed solid. Hope this helps

Can the shower head be tilted?

Yep. Sits in a ball socket so can tilt . Not huge but enough.
