It may vary with different versions of the top, I suppose. I got the boy version without any bows or anything. I am 34 weeks pregnant and weigh around 192lbs. I'm 5'2. The medium fits, it is just a bit tighter and definitely not as a long as I expected from what the reviews read. I also didn't pay enough attention to notice it is 100% cotton and will shrink. So I definitely can't wash and dry it before my party. It's like 8 inches longer than a normal top so maybe that's where they figure it's maternity. It's snug so it doesn't have the side elastic/rouching you expect most maternity tops to have. I bought a large & it fits snugly. It's long enough to be a mini dress practically which means I'm rolling it under to wear with pants. It serves its purpose for Halloween pictures.
