Can this buckle accidentally get released by one side of the mechanism getting actuated rather than both sides?

I've never had both sides come undone, when one side mechanism is pushed. Usually, I'd have to rehook both sides to release the buckle.

I received only the belt, without the keychain, can a replacement be sent or do I have to return the product?
