I don't recall, probably China, but they did not have any odd or chemical smells. They are a beautiful color and great quality. I am happy with this purchase.

Walmart- just get some cheap $5 pillows or Big Lots or Walmart . ..don't pay more for inserts- just get some cheap pillows- any color

I guess you can if you take your cushion cover to a store to make sure it's the same time of velvet or check if the Amazon store you brought the covers sales the fabric by the yard or try to find the fabric in the internet, that's what I will do.
