I'm not able to offer any combo package prices currently due to an overwhelming demand since the beginning of the pandemic. You would need to order each package separately. Thanks for asking.

What's the dimension of the hole? What thickness of bolts would I need?

The bolts most people use are called hex head or allen head bolts, and the holes are set up for 3/8" bolts secured with t-nuts. They can be purchased in a variety of lengths from most home improvement stores (Home Depot, Lowes, Ace Hardware, etc.). I don't include the hardware since you can buy it as cheaply as I can, and I'd have to mark it up a little to cover my costs. Since I use real rock the thickness of the holds does vary, and I recommend waiting until you have the holds to purchase the bolts. Let me know if you have any other questions, thanks.

Are the rocks surfaced to provide a flat side to attached to the wall?

Not exactly flat, but pretty close. I have a flat wall, and have had no problems
