Is this an always on system? Or is it activated by putting the vehicle in reverse?

It is activated when you put the vehicle in reverse.

Does it turn off with the car?

Yes it does. In fact it draws power from the reverse light circuit. So when you're not in reverse, there's no power to the unit anyway.
